Treatment Overview

Direct-to-implant mastectomy replaces the traditional two-step reconstruction process with a single streamlined surgery. Instead of waiting months with a temporary tissue expander, you receive your implant right away – during the very same procedure as your mastectomy.

This approach is often offered to patients who fit specific criteria and are usually also candidates for nipple-sparing mastectomy, though other patients may be eligible, too. You may also be a candidate for preserving your breast sensation, which can be performed with nerve grafts during the same surgery. Our plastic and breast surgeons will walk you through your options and help determine the best path for your anatomy and goals.

About a week before surgery, we'll schedule a pre-admission visit to:

Review what will happen during your surgery
Review potential side effects
Discuss medications and post-surgery care, including in-home help or equipment if needed
Discuss your realistic, aesthetic reconstructive outcomes

You'll receive information to prepare for your hospital admission. If you have any questions after your pre-admission appointment, your nurse or care coordinator will be happy to help.

Direct-to-implant surgery typically takes several hours, but you’re usually able to go home the same day. You may have soreness, swelling and bruising, along with temporary drains to prevent fluid buildup. Patients usually feel back to themselves in a few weeks, but exercise restrictions are in place for 4–6 weeks (specifically lifting and strenuous activity). Your care team will check in regularly as you heal.

Direct-to-implant mastectomy means fewer surgeries and a shorter overall timeline than reconstruction with tissue expanders, since there's no separate expansion process or surgery to plan around. For many patients, waking up from a mastectomy with reconstruction already in place brings real emotional relief, easing the transition through one of the harder parts of treatment. It can also mean fewer appointments and less recovery time overall. There is, however, always the possibility of needing a tissue expander if, during the surgery, your surgeon feels it’s safer to undergo a two-stage process instead.

As with any surgery, there are risks, including:

Infection
Bleeding or fluid buildup
Delayed wound healing
Implant malposition or asymmetry
Capsular contracture (scar tissue tightening around the implant)
Skin or nipple numbness
Scarring that may be thickened or raised

Your care team will talk with you about these risks, answer your questions and help you make an informed decision that aligns with your goals.

You’ll have several follow-up appointments after your procedure for the first few weeks. This helps us monitor your healing and manage any complications. As time goes on, your appointments will be less frequent, but regular checkups are required so we can continue to manage any of your ongoing treatment needs.

It can. If you're likely to need radiation after surgery, your surgical team will factor that into your reconstructive plan. Radiation can increase the risk of complications like scar tissue tightening around the implant, infection, and wound-healing issues. This doesn't rule out direct-to-implant reconstruction, but it's an important conversation to have with your plastic surgeons before your surgery, so your treatment plan accounts for it.

Yes, sensation preservation can often be performed during the same surgery as a direct-to-implant mastectomy to help preserve or restore sensation in the nipple-areola complex and surrounding breast tissue. It's a collaborative process between your breast and plastic surgeons and can be a useful addition if you are deemed to be a good candidate for this approach.

They certainly can! However, every patient heals differently, and results depend on factors like your skin envelope, body type, skin elasticity and whether you need additional treatments like radiation.
